・12・ 中国口腔种植学杂志2011年第16卷第1期 ation of critical—sized calvarial defects was observed in the groups which were implanted with two transgenic BMSCs and B—TCP coin— posites.And in these experimental groups,bone areas and vascular densities were increased signiifcantly(P<0.05)than other groups.In conclusion,sequential delivery of angiogenic and osteogenic factors likely has a synergistic effect,mimicking the molecular events of natural bone regeneration. 【Keywords】tissue engineering,basic ifbroblast growth factor,sonic hedgehog,bone marrow—derived mesenchymal stem cells,tet— racycline regulated system. 种植修复在牙周病治疗中的应用 徐欣 (山东大学口腔医学院种植中心) 牙周病中的骨量不足与种植修复:因牙周病的类型、拔牙的时间、治疗的效果、患者的体质及自我维护等因素的不同而千 姿百态,牙周病患者的牙槽骨也具有了显著的特点。一直以来总有些学者认为牙周炎患者进行种植修复可能会显著增加种 植失败率。从我们的临床工作,得到了以下的结论:1.在伴有骨量不足的牙周病患者中进行种植治疗可以获得良好的效果。 2.牙周病患者种植治疗受多因素的影响,需要全面考虑并特殊对待。3.牙周病种植的设计应该是菜单式的选择,同时应体现 客观性和前瞻性。 Dental implant restorations in periodontally involved patients with insuficient bone volume.f XU Xin (Implant Center of Stomatological School,Shandong University) Due to the etiology of the periodontal conditions,timing of the extraction,effectiveness of the treatments,patient’S physical con— dition and oral hygiene maintenance,Typical symptoms were shown in periodontally involved patients.Traditionally some researchers believe dental implant installed in periodontally involved patient will increase failure rate signiifcantly.From our clinical work,we get conclusions from these patients:1.Successful restoration using dental implants can be achieved in periodontally involved patient with bone volume deficiency.2.Special considerations and systemic considerations are needed due to multiple factors involved when treating periodontally involved patients with dental implants.3.Treatment planning in periodontally involved implant cases should be in a se— lective menu format.It should relfect objective considerations and proactive forward planning. 盐酸米诺环素软膏治疗种植体周围炎的临床疗效观察 陈 岭 (烟台市口腔医院) 【摘要】 目的:探讨和评估盐酸米诺环素软膏在治疗种植体周围-Ecs临床疗效。方法:选择种植体周围炎患者20名(共23 枚种植体),X线示种植体周围骨质吸收范围为种植体1/3~1/2。所有患者进行龈上洁治、龈下舌11治,局部给予盐酸米诺环素 软膏。一次/周。观察治疗前,治疗后第1~6周种植体周围的茵斑指数(PLI)、出血指数(sBI)和探诊深度(PD)等指标的变 中国口腔种植学杂志2011年第16卷第1期 ・l3・ 化。结果:除治疗后第2周,一枚种植体发生脱落外,6周后,其余种植体稳定性均良好,检测菌斑指数、探诊出血指数和探诊 深度有明显改善,x线示未见明显继续骨质吸收。结论:局部应用盐酸米诺环素软膏对种植体周围炎有一定的治疗效果,远期 效果应进一步观察。 【关键词】种植体周围炎;盐酸米诺环素软膏 Effects of Applying Ointment of Minocycline HydrOchl0ride for peri—implantitis CHEN Ling 【Abstract】 Objective:To evaluate the clinical eicacyf of applying ointment of minocycline hydrochloride to treat peri—implantitis. Methods:Choose 20 patients with peri—implantitis(23 implants、.X—ray showed 1/3—1/2 bone arounded implants has lossed.All patients were taken supragingival scaling and subgingival scaling.,local application of Minocycline Hydrochloride once a week.Param— eters including plaque index(PLI),sulcular bleeding index(SBI)and probing depth(PD)of pockets were measured at beginning of the treatment 1~6 weeks after treatment.Result:Except one implant fails after treatement 2 weeks,the others have good stability.The methods significantly decreased PLI、SBI and PD.X—ray showed bone didn’t loss.Conclusion:The local application of Periocline is a signiicantf effect for dental peri—implantitis and long—term following up needed. 【Key words】peri—implantitis;minocycline hydrochloride 重度牙周炎患者种植总义齿修复的临床观察 陈 刚 马练 李树春刘学恒 (北京大学深圳医院口腔科) 【摘要】 目的:探讨重度牙周炎患者拔除余留牙后行种植总义齿修复的临床疗效。材料和方法:重度牙周炎患者18位,拔 除单颌所有余留牙后即刻或4~6周后植入种植体,采用即刻负重或延期负重的修复方式,并于术后3~8月后完成最终种植 总义齿的修复。对所有患者进行临床及x线检查评价修复效果。结果:18位患者共植入144颗种植体,共完成24个种植总 义齿的修复,其中上颌14例,下颌10例,12例采用了即刻负重的方式,l2例3~8个月后延期负重。最终9例行固定总义齿 修复,l5例行覆盖义齿修复。永久修复完成后随访10~36个月,平均13个月。观察期内共2颗种植体丧失。修复完成后6 个月时与植入时相比,种植体周边缘骨吸收0.56ram。采用覆盖义齿或固定义齿,种植体边缘骨吸收无明显差异。讨论:对于 临床预后不佳的重度牙周炎患者,早期拔除余留牙后采用种植总义齿的修复方式,可早日恢复患者的咀嚼功能。严格的适应 症选择,合理的治疗设计及正确的手术实施是保证成功率的关键。结论:对重度牙周炎患者采用种植总义齿的修复方式可获 得满意的临床效果。 Clinical evaluation of implant——supported full dentures or patients sufffered severe periodontitis CHEN Gang,MA Lian,LI Shuchun,LIU Xueheng (Department of Stamatology,Peking University,Shenzhen) [Abstract]Purpose:To evaluate the eicacfy of implant—supported fu11 dentures for the treatment of severe periodontitis patieuts Material and methods:All of the remaining teeth in upper or lower jaw were extracted in 1 8 patients suffered severe periodontitis I plants were inserted immediately or 4 t。6 weeks after teeth extractionImmediate or delayed loading were used for all the patients .
